PawKet Vet Consult Referral Form Cardiology-Only Referrals Pet Portal

At EVG, our outpatient imaging services are specifically designed to facilitate ultrasounds for patients referred by veterinarians who determine that a full internal medicine consultation is unnecessary. Our goal is to provide a seamless and efficient imaging experience, offering comprehensive ultrasound reports that include detailed differential diagnoses, further diagnostic recommendations, and/or suggested treatments. Questions about the report findings or needing more clarity as to next steps? We encourage vet-to-vet discussions for further clarification and feedback on the ultrasound findings and don’t want you to hesitate to reach out. In fact, we may have already called you!

We understand the importance of timely access to diagnostic imaging, which is why we offer same-day or next-day appointments to accommodate the needs of our referral partners. At the conclusion of the imaging appointment, both the pet parent and the referring veterinarian will receive the ultrasound report, ensuring immediate access to critical information.

Our diagnostic imaging team will meet clients at intake to ensure a smooth and welcoming process. To optimize the ultrasound evaluation, we request that referring veterinarians provide a brief medical history and the reason for the imaging, along with any specific areas of focus. Please submit our diagnostic imaging request form to info@endeavourvets.com. Appointments are by referral only. Please contact us at 803.708.0162 to discuss availability.
